Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Our commitment
The Office of Steffi Mendoza, M.D. is required by law to protect the privacy of your health information, to give you this notice of our legal duties and privacy practices, to follow the notice currently in effect, and to tell you if there is a breach of your unsecured health information.
How we may use and share your health information
- Treatment: to provide your care and coordinate with other providers involved in it (for example, a pharmacy or your pediatrician).
- Payment: to bill for services and prepare superbills you can submit to your insurance. We are an out-of-network, private-pay practice.
- Health care operations: to run the practice, for example scheduling, quality review, and record keeping.
- When the law requires or allows it: for example, to prevent a serious threat to health or safety, for required reports of suspected abuse or neglect, to respond to court orders, or for public health and health oversight activities.
Uses that need your written permission
- Most sharing of psychotherapy notes
- Marketing, and any sale of your information
- Any other use not described in this notice
You can cancel a permission you’ve given at any time, in writing. Cancelling won’t undo anything already shared while the permission was in place.
Minors
For patients under 18, parents or guardians generally make privacy decisions for their child. State law (Texas or Colorado) may give minors rights over some types of care and records.
Your rights
You have the right to:
- See or get a copy of your health records (a reasonable, cost-based fee may apply)
- Ask us to correct information you believe is wrong or incomplete
- Ask us to limit what we use or share. We aren’t required to agree in every case, but we must agree not to share with your health plan for a service you paid for in full out of pocket, if you ask.
- Ask us to contact you confidentially (for example, at a different number or address)
- Get a list of certain disclosures we’ve made
- Get a paper copy of this notice at any time
- Choose someone to act for you, such as a legal guardian or someone with medical power of attorney

How to complain
If you believe your privacy rights have been violated, you can:

We will not retaliate against you for filing a complaint.
Changes to this notice
We may change this notice. Any change will apply to all information we hold. The current notice will be posted on our website and available at our office.
